School Sports & Camp Physicals

Getting a sports physical or a pre-summer camp check-up helps determine whether it is safe for your child to participate in a specific sport or activity.

Participation in Sports, Band, or Camps requires an Evaluation of the Following:

  • Your family's complete medical history, including any serious illnesses, heart disease, asthma, or diabetes
  • Allergies
  • Past surgeries or hospitalizations
  • Blood Pressure
  • Vision
  • Height and Weight
  • Joints, posture, and flexibility

Clearance for Sports, Band, and Camp Physicals

Patients will need to provide clearance letters from their physician(s). Vision screening will be performed. If you wear glasses daily, bring them.

Conditions for which you will need a clearance letter from your physician include but are not limited to:

 

  • Existing heart condition
  • Concussions within the last two years
  • Broken bone within the last year
  • Surgery (or surgeries) within the previous year
  • Scoliosis -in addition to the clearance letter, bring a recent x-ray with a degree of curvature (if diagnosed)

Preparing for a sports and summer camp physical

Before your appointment, make sure to bring any forms that the healthcare provider needs to fill out. Some states require sports clearance exams before camps or sports, which might need specific documents. Check everything you need to bring before going to our urgent care for your physical.
